Billing and Insurance

I accept the following forms of payment:

  • Major Credit Cards

  • HSA

  • Ecclesiastical Pay/Reimbursement

Insurance:

Effective April 1, 2023, I am paneled with and accept the following insurances:

  • Aetna

  • EMI

  • UnitedHealthCare (UHC)

  • SelectHealth (Choice, Med, Care, Value, Share, CHIP)

  • Regence Blue Cross Blue Shield of Utah

It is the client's responsibility to verify coverage. Clients are responsible for payment of services if insurance does not pay. We do not accept Medicaid at this time.

Each clinician may accept different insurances.

Court communication and letter writing require additional fees that are not covered by insurance.
